This form is only to be used if you have lost or forgotten your MyTCS password and need your password to be reset. You can also use this form to request your account be unlocked in the event that it has become locked due to the fact that someone attempted to login with an invalid password more than 10 times in a row.

A new password will be emailed to you within three business days.
