2011 Re-Enrollment Process for Current Familes

Re-Enrollment Begins
  Re-enrollment fee $150 ($450 Family Maximum) Non-Refundable
   
  Email sent to all families with personalized link for online re-enrollment.
 
   
 

All online re-enrollment completed before March 1st will be entered for the early re-enrollment incentive drawings. The earlier you enroll the more changes you have to win. Give away include, 3 IPADS, 3 Cedar Point family packs, and 3 Toledo zoo family memberships.

Febuary 1st 1st early enrollment incentive drawing.
Febuary 15th 2nd early enrollment incentive drawing.
March 1st 3rd early enrollment incentive drawing.

Early Re-enrollment ends / Priority scheduling for re-enrollment ends.
  All additional and applicable forms must be turned in prior to the first day of school.
These forms can be found in the Facts Online Re-enrollment or on the school web site at www.toledochristian.com under For Parents > Online forms
 
  • Custody Agreements (If changed during Past year)
  • Kindergarten Physician Dental Form (Need to have if complete only if you have a Re-enrolled child entering Kindergarten)
  • Emergency Medical Form
  • Physician Statement of Prescription Medication (This form must accompany the medication)
  • Volunteer Driver Form (Parents and Student Drivers)
  • Physical Form for Athletic Participation (7th thru 12th Grades) Download from link provided
  • Fitness Center Waiver for Athletic Participation (7th thru 12th Grades)
  • Trip Registration Form ( Optional)
